Hemp and Clopidogrel

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Hemp interacts with whole categories of medication. Clopidogrel falls into 3 of those categories, so it can interact with Hemp in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Clopidogrel

Clopidogrel falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the quick version: Plavix (clopidogrel) is a blood thinner that keeps your platelets from clumping, which lowers your risk of clots after a stent, heart attack, or stroke. The thought here is that hemp seed might also nudge platelets in that same direction, so in theory the two together could add up and make bleeding a little more likely (think easier bruising, nosebleeds, or bleeding gums).

But the evidence is very thin. The only signal comes from a lab and animal study using hemp at high amounts. When real people took hemp seed oil (2 grams a day for 12 weeks), it did not measurably affect platelets. So everyday hemp foods are very unlikely to cause trouble. Still, if you plan to use concentrated hemp supplements, check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Clopidogrel also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about Plavix: it's a prodrug, meaning your body has to switch it on before it can do its job of keeping platelets from clumping and forming clots. That activation happens mostly through a liver enzyme called CYP2C19, not the CYP1A2 enzyme this hemp interaction is about. So even though lab studies suggest hemp might speed up CYP1A2 a little, that's not the main pathway that turns clopidogrel into its active form.

Bottom line, this is a minor, theoretical concern built only on test-tube research, and food amounts of hemp seeds or hemp oil are very unlikely to change how well your clopidogrel works. Still, because this drug protects against strokes and heart attacks, it's worth a quick check with your pharmacist or doctor before starting concentrated hemp supplements.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Clopidogrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Here's the thing about Plavix (clopidogrel): it's actually a prodrug, which means it has to be switched on by your liver enzymes before it can thin your blood and protect your heart. Most of that activation runs through CYP2C19, but CYP3A4 plays a role too. Lab studies suggest hemp might gently speed up (induce) CYP3A4.

Now, you might expect speeding up an enzyme to make a prodrug work better, but this is a complicated, two-step activation, so the real-world effect on Plavix isn't clear cut. Honestly, this is a minor, theoretical concern based only on test-tube research, not on people. Food amounts of hemp seeds or hemp oil are very unlikely to matter. If you're thinking about concentrated hemp supplements while on Plavix, just run it by your pharmacist or doctor first to be safe.

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.

About Clopidogrel

What Is Clopidogrel Used For?

Brand name: Plavix

Clopidogrel tablets are used to reduce the risk of heart attack and stroke in people with acute coronary syndrome (ACS) — including unstable angina and non-ST-elevation or ST-elevation heart attacks — and in people with a history of recent heart attack, recent stroke, or established peripheral arterial disease. For ACS, it is typically given alongside aspirin.

It works by preventing platelets (blood cells that trigger clotting) from clumping together, which helps keep arteries open and lowers the chance of a dangerous clot forming.
