Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the piece that matters with this combo pill: the omeprazole part is cleared by a liver enzyme called CYP2C19. Lab and animal studies suggest schisandra can slow that enzyme down. In theory, that means omeprazole could hang around a bit longer and have a somewhat stronger effect. Since omeprazole is already there to protect your stomach, that's usually not dangerous, but higher levels over time can occasionally bring on things like headache, stomach upset, or diarrhea.

The aspirin side works differently and isn't really affected by this enzyme. Importantly, this interaction has only been seen in test tubes and animals, never proven in people, so it's more of a caution than a real alarm. Still, it's worth a quick word with your pharmacist or doctor before adding schisandra.

Here's the gist with Yosprala (the aspirin plus omeprazole combo). In lab dishes and animal studies, schisandra seems to speed up a liver enzyme called CYP2C9. Omeprazole is partly handled by that enzyme, so in theory schisandra could clear it a little faster and make it work less well. If that happened, your stomach might make more acid than the medicine is meant to control, which could nudge up your risk of an aspirin-related ulcer or stomach upset.

I want to be honest, though: this has never actually been shown in people, so it's more of a theoretical concern than a proven problem. The aspirin part isn't really affected here. Before adding schisandra, just run it by your pharmacist or doctor so they can keep an eye on things.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the short version for Yosprala (the aspirin plus omeprazole pill): schisandra can gently block a liver enzyme called CYP3A4. The omeprazole half of this medicine is partly handled by that pathway, so in theory schisandra could let omeprazole levels drift a little higher. That isn't really dangerous, since omeprazole is well tolerated, but you might notice things like headache or stomach upset if levels rise. The aspirin portion isn't cleared this way, so its blood-thinning effect shouldn't change much here.

Honestly, this concern comes mostly from lab and animal work, not real patients, so I wouldn't lose sleep over it. Still, since you're taking this for heart and stroke protection, run schisandra by your pharmacist or doctor before adding it.
