Cabotegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the situation with Cabenuva, which is the long-acting injection combining cabotegravir and rilpivirine. Rilpivirine in particular is partly cleared by a liver enzyme called CYP3A4. Schisandra tends to slow that enzyme down, so in theory it could let rilpivirine build up a bit and slightly raise the odds of side effects like headache, trouble sleeping, mood changes, or feeling dizzy.

That said, the evidence here is weak. This concern comes mostly from lab and animal work, not from real people taking these two together, so I don't want to overstate it. Since Cabenuva is given as a scheduled injection for HIV, please don't change anything on your own. Just run the schisandra past your HIV pharmacist or doctor first so they can weigh in.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
