Cabotegravir, Rilpivirine falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the concern with this combination. The rilpivirine part of Cabenuva can, on its own, stretch out the heart's electrical recovery time (what we call the QT interval). Ephedra contains stimulants like ephedrine that can do something similar and also rev up your heart rate and blood pressure. Put the two together and you may add one effect on top of the other, which can raise the risk of a dangerous irregular heartbeat.

This is largely a theoretical, additive concern, but it's worth taking seriously since the heart rhythm is involved. Keep in mind that ephedra is actually banned in U.S. supplements because of its heart risks, so I wouldn't recommend it regardless. Please talk with your pharmacist or doctor before using any ephedra-containing product.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
