Schisandra and Lansoprazole

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Schisandra interacts with whole categories of medication. Lansoprazole falls into 2 of those categories, so it can interact with Schisandra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 2

Why this applies to Lansoprazole

Lansoprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the gist with Prevacid (lansoprazole) and schisandra. Your body relies on a liver enzyme called CYP2C19 to break lansoprazole down and clear it out. Schisandra, in lab and animal testing, seems to slow that enzyme down. If that holds true in people, lansoprazole could hang around a bit longer and build up, which might mean slightly stronger effects or more side effects like headache, diarrhea, nausea, or stomach upset.

I want to be honest about the strength here: this is theoretical. It has only been seen in test tubes and animals, never confirmed in humans, and lansoprazole is a generally forgiving, well-tolerated drug. So I wouldn't lose sleep over it. Still, if you plan to use schisandra regularly, give your pharmacist or doctor a quick heads-up first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Lansoprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Prevacid (lansoprazole): your body clears it partly through a liver enzyme called CYP3A4 (along with another one, CYP2C19). Schisandra tends to slow CYP3A4 down, so in theory more lansoprazole could linger in your system. Lansoprazole is not a prodrug, so more drug simply means a bit more of its usual effect, and possibly side effects like headache, nausea, diarrhea, or stomach upset.

The good news: this is a moderate, mostly theory-based concern from lab and animal research, not real-world human reports with this exact pairing. Lansoprazole also has a wide safety margin, so a small change is unlikely to cause trouble. Still, if you plan to take both regularly, run it by your pharmacist or doctor first.

About Schisandra

What is Schisandra?

Schisandra
  • Fatigue and low energy
  • Stress and adaptogen support
  • Liver health
  • Mental focus and performance
  • Menopause symptoms
  • Respiratory complaints

Schisandra is a traditional Chinese medicine berry used as an adaptogen for stress, fatigue, and liver support. Human evidence is limited and most claims are not well proven, but it appears reasonably well tolerated by most healthy adults when used short term. Pregnant and breastfeeding people should avoid it, and anyone with a health condition should check with a pharmacist or doctor first.

About Lansoprazole

What Is Lansoprazole Used For?

Brand name: Prevacid

Lansoprazole is prescribed for a range of conditions where excess stomach acid is the problem. It treats active stomach and duodenal ulcers (sores in the stomach or upper small intestine), helps heal ulcers caused by NSAID pain relievers, and can reduce the risk of those ulcers coming back.

It's also used for gastroesophageal reflux disease (GERD), erosive esophagitis (acid damage to the esophagus), and rare conditions where the stomach makes far too much acid — like Zollinger-Ellison syndrome. When combined with antibiotics, it helps wipe out H. pylori bacteria that cause recurring ulcers.
