Why this applies to Lansoprazole

Lansoprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grapefruit interacts with.

Grapefruit juice can increase levels of drugs metabolized by CYP3A4.
Clinical research shows that grapefruit juice can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. When taken orally, effects of grapefruit juice on CYP3A4 levels appear to last at least 48 hours. Grapefruit's ability to inhibit CYP3A4 has even been harnessed to intentionally increase levels of venetoclax, which is metabolized by CYP3A4, in an elderly patient with acute myeloid leukemia who could not afford full dose venetoclax. The lower dose of venetoclax in combination with grapefruit juice resulted in serum levels of venetoclax in the therapeutic reference range of full dose venetoclax and positive treatment outcomes for the patient.
Professional consensus recommends the consideration of patient age, existing medical conditions, additional medications, and the potential for additive adverse effects when evaluating the risks of concomitant use of grapefruit juice with any medication metabolized by CYP3A4. While all patients are at risk for interactions with grapefruit juice consumption, patients older than 70 years of age and those taking multiple medications are at the greatest risk for a serious or fatal interaction with grapefruit juice.

Here's the good news on this one: grapefruit and lansoprazole (Prevacid) are not a worrisome pair for most people. The interaction in our file is about grapefruit slowing down a liver enzyme called CYP3A4, which can let certain drugs build up. Lansoprazole isn't really a CYP3A4 drug, though. Your body clears it mainly through a different enzyme (CYP2C19), so grapefruit doesn't have much grip on it.

If anything, a small amount more lansoprazole might hang around, but it's a forgiving medicine, and side effects like headache, mild nausea, or loose stools are uncommon and minor. Enjoy your grapefruit. Still, if you take several medications or are over 70, it's smart to run your full list past your pharmacist, since grapefruit matters a lot for some other drugs.

Lansoprazole also falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES.

Theoretically, grapefruit juice might increase levels of drugs metabolized by CYP2C19.
In vitro research suggests that grapefruit juice might inhibit CYP2C19 enzymes. Also, a small clinical study shows that taking grapefruit juice with clopidogrel, an antiplatelet prodrug that is metabolized primarily by CYP2C19, decreases plasma levels of the active metabolite and impairs the antiplatelet effect of clopidogrel. This effect is likely due to grapefruit-induced inhibition of CYP2C19.

Here's the basics: lansoprazole (Prevacid) is cleared from your body partly by a liver enzyme called CYP2C19. Grapefruit can mildly slow that enzyme down, so in theory your body might break down lansoprazole a bit more slowly and let levels run a little higher. The good news is that lansoprazole is the active drug itself, not a prodrug, and it has a wide safety margin, so a small bump usually just means a touch more of its usual effects.

This concern is mostly theoretical and based on lab studies, not strong human data with lansoprazole. Occasional grapefruit at meals is unlikely to cause trouble. If you drink it daily or notice things like headache, nausea, or belly upset, check with your pharmacist or doctor first.

What Is Lansoprazole Used For?

Brand name: Prevacid

Lansoprazole is prescribed for a range of conditions where excess stomach acid is the problem. It treats active stomach and duodenal ulcers (sores in the stomach or upper small intestine), helps heal ulcers caused by NSAID pain relievers, and can reduce the risk of those ulcers coming back.

It's also used for gastroesophageal reflux disease (GERD), erosive esophagitis (acid damage to the esophagus), and rare conditions where the stomach makes far too much acid — like Zollinger-Ellison syndrome. When combined with antibiotics, it helps wipe out H. pylori bacteria that cause recurring ulcers.
