Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the gist for Fycompa (perampanel). Your liver uses several enzymes to clear this seizure medicine, and one of them is called CYP2C19. Schisandra has been shown, mostly in lab dishes and animal studies, to slow that enzyme down. In theory, that could let perampanel build up a little higher than usual in your body.

If that happened, you might notice more of perampanel's usual side effects, like dizziness, drowsiness, trouble with balance, or feeling irritable or moody. I want to be honest though: this has not actually been seen in people, so it's a theoretical concern, not a proven one. Perampanel also isn't a prodrug, so the worry is too much effect, not too little. Before adding schisandra, please run it by your pharmacist or doctor first.

Here's the gist for Fycompa (perampanel). Your body relies partly on a liver enzyme called CYP2C9 to handle this seizure medicine. Lab and animal studies hint that schisandra may speed that enzyme up, which could clear perampanel out a little faster and leave less of it working in your system. In theory, that might weaken seizure control, and for a seizure medicine that's the worry, a breakthrough seizure.

I want to be honest about how thin this evidence is. This effect has only shown up in test tubes and animals, never confirmed in people, so it's mostly theoretical. Still, because losing seizure protection is a big deal, please loop in your neurologist or pharmacist before adding schisandra rather than starting it on your own.

Perampanel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
