Eurycoma Longifolia and Mavacamten

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Camzyos (mavacamten). Your body clears this medicine mainly through a liver enzyme called CYP2C19. In a test-tube study, tongkat ali appeared to weakly block that enzyme. If that happened in real life, your body might clear mavacamten a little slower, letting levels creep up. Since mavacamten works by relaxing the heart muscle, higher levels could push too far and weaken the heart's pumping, which can cause shortness of breath, tiredness, swelling, or dizziness.

That said, this is only a theory from lab work and has never been seen in people, so I wouldn't lose sleep over it. Still, mavacamten is a carefully dosed heart drug, so please run any supplement past your cardiologist or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Mavacamten page

“Grapefruit is the first thing I would set aside for anyone taking mavacamten (Camzyos), the medicine for obstructive hypertrophic cardiomyopathy. It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”

About Mavacamten

What Is Mavacamten Used For?

Brand name: Camzyos

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.

The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
