Esomeprazole Strontium falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist: your body uses an enzyme called CYP2C19 to help clear esomeprazole, which is the acid-reducing medicine in this product. In lab dish testing, tongkat ali appeared to weakly slow down that same enzyme. In theory, that could let a little more esomeprazole linger in your system, slightly nudging up its effect or side effects like headache, nausea, or stomach upset.

The reassuring part: this has only been seen in test tubes, never in actual people, and the interaction is rated minor. For most folks it's unlikely to cause any noticeable problem. Still, since supplement quality varies a lot, it's worth giving your pharmacist or doctor a heads-up before combining the two so they can keep an eye on things.
