Clomipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.
In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body uses a liver enzyme called CYP2D6 to help break down clomipramine. In test-tube studies, quercetin slows that enzyme down. If that same thing happened in people, clomipramine could clear more slowly and build up a bit in your system, which can mean stronger effects and more side effects like dry mouth, constipation, drowsiness, dizziness, a fast heartbeat, or trouble urinating.
But here's the important context: this has only been seen in the lab, not in actual people, so we really don't know if it matters in real life. The amount of quercetin in everyday foods is very unlikely to be a problem. If you're considering a high-dose supplement, just run it by your pharmacist or doctor first so they can keep an eye on things.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the gist: quercetin in concentrated supplement form may slow down one of the liver enzymes (CYP2C9) that helps clear clomipramine from your body. If that enzyme is partly blocked, more clomipramine could hang around longer, which can nudge up its usual effects and side effects, things like dry mouth, constipation, drowsiness, dizziness, a faster heartbeat, or trouble urinating.
I want to be straight with you, though: this is mostly theoretical for clomipramine. The clearest evidence comes from a small study using a different drug, and clomipramine is also handled by other enzymes. Food amounts of quercetin are unlikely to matter, but if you're considering high-dose capsules, run it by your pharmacist or prescriber first so they can watch for any added side effects.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to help break down clomipramine, and quercetin can nudge that enzyme. The tricky part is which way it goes. In most lab studies quercetin seems to block CYP3A4, which could let clomipramine build up a bit and bring on more of its usual side effects, things like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat. But one human study showed the opposite, with quercetin actually speeding that enzyme up, which could weaken the drug.
So honestly, the direction isn't clear, and this is based mostly on lab and animal work, not solid human studies on clomipramine itself. Food amounts of quercetin are very unlikely to matter. Before starting a concentrated supplement, just run it by your pharmacist or doctor.
