Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) and goldthread. Goldthread contains berberine, which lab studies hint might slow down a liver enzyme called CYP3A4. Part of your Vosevi combo, the voxilaprevir piece, is handled by that enzyme. If berberine slows it, levels of the drug could drift a little higher than expected, which in theory might mean more of its usual side effects like headache, tiredness, nausea, or diarrhea.

I want to be honest about how shaky this is. It's only a theoretical concern from test-tube work, never actually reported in people, and food amounts wouldn't be the issue, concentrated supplements would. Since you're treating hepatitis C, please loop in your pharmacist or doctor before adding goldthread rather than changing anything on your own.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
