Here's the gist of the concern with Vosevi (sofosbuvir, velpatasvir, voxilaprevir) and adrafinil. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP1A2. In theory, speeding up that enzyme could lower the levels of certain medications it processes. The worry on file is that this might make a hepatitis C combo slightly less effective.

Honestly, this is a theoretical flag based on lab data, not real-world cases, and these three antivirals aren't mainly cleared by CYP1A2. So the practical risk looks low. Still, because successfully clearing hepatitis C matters so much, and adrafinil itself is hard on the liver, I'd run this by your doctor or pharmacist before combining them.

Here's the gist: when your body turns adrafinil into modafinil, that modafinil can gently speed up a liver enzyme called CYP3A4. Voxilaprevir (one of the three drugs in Vosevi) is partly handled by that enzyme. So in theory, adrafinil could nudge your body to clear it a little faster, which might lower its levels and could make your hepatitis C treatment work less well. With these cure-focused regimens, you really want full drug levels for the whole course.

I want to be honest, though: this concern is theoretical, based on how the drugs behave in the lab, not on real patient reports. The effect on CYP3A4 is also described as weak. Still, because adrafinil is unregulated and rough on the liver (and you're already treating your liver), please run it by your doctor or pharmacist before mixing them.

Here's the short version on combining adrafinil with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir). Once your liver turns adrafinil into modafinil, that modafinil may very mildly speed up an enzyme called CYP2B6. In theory, speeding up an enzyme that helps clear a medicine could lower that medicine's levels and make it work less well. For a hepatitis C treatment, the worry would be slightly less virus-fighting power.

That said, this is a theoretical concern based on lab data, not real patient reports, and the effect on this enzyme is weak. I'd still flag two bigger-picture things: Vosevi is a serious 12-week treatment you don't want to undercut, and adrafinil is hard on the liver, which is already your concern with hepatitis C. Please run this by your doctor or pharmacist before mixing them.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
