Why this applies to Ivabradine

Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your ivabradine. Your liver uses an enzyme called CYP3A4 to clear ivabradine out of your system. Goldthread's main compound, berberine, may slow that enzyme down. If it does, ivabradine could build up to higher levels than expected, which can push your heart rate too low and make side effects more likely, things like dizziness, lightheadedness, tiredness, or those brief visual brightness flashes (called phosphenes) that ivabradine is known for.

The good news: this is only a theoretical concern from lab pharmacology, and it hasn't actually been reported in people. Still, since ivabradine already deliberately slows the heart, I'd play it safe. Please check with your pharmacist or doctor before pairing these.

About Ivabradine

What Is Ivabradine Used For?

Brand names: Procoralan, Coralan, Corlentor, Corlanor

Ivabradine (including the brand Corlanor) is used in adults with stable, symptomatic chronic heart failure with reduced pumping function to reduce the risk of being hospitalized for worsening heart failure. It is prescribed when the heart is beating too fast at rest and the patient is either already on the highest tolerated dose of a beta-blocker or cannot take one.

Corlanor is also approved to treat stable, symptomatic heart failure caused by dilated cardiomyopathy (an enlarged, weakened heart muscle) in children ages 6 months and older who have an elevated heart rate.
