Atazanavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for your Evotaz, which combines atazanavir with a booster called cobicistat. Both pieces of this medicine are handled by a liver enzyme known as CYP3A4. Some lab studies suggest melatonin might mildly slow that same enzyme, which in theory could nudge atazanavir levels up a bit. Higher atazanavir can mean more of its typical effects, like yellowing of the skin or eyes (a harmless rise in bilirubin), upset stomach, or headache.

The good news: this is a minor, theoretical concern based only on test-tube research, not real patient reports, so I wouldn't lose sleep over it. Short-term melatonin for jet lag or sleep is usually fine, but since HIV medicines matter, just give your pharmacist or doctor a quick heads-up before starting.

What Is Atazanavir and Cobicistat Used For?

Brand name: Evotaz

Evotaz (atazanavir/cobicistat) is used to treat HIV-1 infection. It must always be taken together with other antiretroviral medicines — it is not a complete regimen on its own. It is approved for adults and for pediatric patients who weigh at least 35 kg.

In patients who have been on HIV treatment before (treatment-experienced), how well Evotaz works depends on how much resistance to protease inhibitors the virus has already developed. Your prescriber will review this before starting you on Evotaz.
