Brexpiprazole falls under ANTIPSYCHOTIC DRUGS — the group Manganese interacts with.
Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Rexulti (brexpiprazole) and manganese. This one isn't about manganese changing your drug levels, and it isn't about the drug being a prodrug. Instead, the concern runs the other way: taking an antipsychotic alongside manganese supplements might raise the risk of manganese building up in the body, which can affect the nervous system.
I want to be straight with you about how thin this evidence is. It's largely theoretical, based on a single older case with a different antipsychotic in someone who also had liver disease. The amount of manganese you get from food is not a worry. Concentrated supplement doses are where I'd pause. Before adding a manganese supplement, just check with your pharmacist or doctor first, especially if you have any liver problems.

What Is Brexpiprazole Used For?
Brand name: Rexulti
Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.
Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
