Upadacitinib Hemihydrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for Rinvoq (upadacitinib). Your body uses an enzyme called CYP3A4 to help clear this medication. Some lab research suggests certain batches of melatonin can mildly slow that enzyme down. In theory, that could let a little more upadacitinib build up in your system, which might nudge up its effects and side effects, like headaches, nausea, or a higher chance of catching infections.

I want to be straight with you, though: this is a minor concern based only on test-tube studies, not on real people. For most folks it likely won't cause trouble. Since Rinvoq already affects your immune system, it's still smart to mention any melatonin use to your pharmacist or doctor so they can keep an eye on you.

Upadacitinib is approved for adults and some children with moderate-to-severe inflammatory conditions when other treatments haven't worked well enough. It targets overactive immune signaling that drives these diseases.

Conditions it treats include:

  • Rheumatoid arthritis (RA) — in adults who didn't respond to TNF blocker medications
  • Psoriatic arthritis — in adults and children 2 years and older who didn't respond to TNF blockers
