Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP2D6.
Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist with this cough and congestion combo (Vicks Formula 44D). The dextromethorphan part is cleared by a liver enzyme called CYP2D6, and some lab studies hint that melatonin might slow that enzyme down a little. In theory, that could let dextromethorphan build up slightly, which might mean a bit more drowsiness, dizziness, or that foggy, lightheaded feeling.

I want to be honest though: this is rated minor and the evidence is only test-tube and animal research, not real-world cases. For most people it isn't a big deal. The bigger overlap is simply that both melatonin and dextromethorphan can make you sleepy, so don't be surprised if you feel extra groggy. If you're unsure or feel off, just check with your pharmacist.

Here's the gist: this cough and congestion combo (dextromethorphan plus phenylephrine) is partly broken down by a liver enzyme called CYP3A4. Some lab research suggests certain batches of melatonin can slow that enzyme down a little. In theory, that could let a bit more of the dextromethorphan linger in your system, which might mean more drowsiness, dizziness, or a foggy feeling, especially on top of melatonin's own sleepy effect.

But please don't lose sleep over this one. The interaction is rated minor and rests only on test-tube data, not real-world cases. For occasional, short-term use it's unlikely to cause trouble. Still, taking these together at bedtime may make you extra groggy, so go easy. If you have questions or take them often, just check with your pharmacist or doctor.
