Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP2D6 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, strongly inhibits CYP2D6 enzyme.

Here's the gist with Rexulti (brexpiprazole) and Acacia rigidula products. Some of these supplements have been found spiked with a stimulant called BMPEA, and lab studies suggest BMPEA can block an enzyme in your liver called CYP2D6. That enzyme is one of the main ways your body clears brexpiprazole. If it's slowed down, the drug can build up in your system.

Brexpiprazole is the active drug itself (not a prodrug), so higher levels usually mean stronger effects and more side effects, things like drowsiness, restlessness, dizziness, or weight gain. That said, this is theoretical and based only on lab testing, so don't panic. Still, given the BMPEA safety worries, please check with your pharmacist or doctor before mixing the two.

In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the situation with Rexulti (brexpiprazole) and Acacia rigidula. Your liver uses an enzyme called CYP3A4 to help clear brexpiprazole out of your body. Some Acacia rigidula products have been found to contain a hidden stimulant called BMPEA, and lab studies suggest BMPEA may slow down that same enzyme. If it does, brexpiprazole could build up a bit higher than expected, which might mean more side effects like drowsiness, restlessness, dizziness, or weight gain.

I'll be honest with you though, this is only theoretical and based on test-tube research, not real patients, so I wouldn't lose sleep over it. The bigger worry is the hidden stimulant itself. Please check with your pharmacist or doctor before mixing the two.

Acacia rigidula is a shrub from Texas and Mexico that is marketed in weight-loss and energy supplements. There is very little reliable human evidence that it works, and some products have been found to contain an unapproved stimulant drug (BMPEA), which raises real safety concerns. What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
