Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for these two. Your liver uses an enzyme called CYP3A4 to clear sildenafil out of your system. Some lab studies suggest melatonin can mildly slow that same enzyme down. In theory, that could let a little more sildenafil linger in your blood, which might nudge up its typical effects like headache, facial flushing, a stuffy nose, or some lightheadedness.

I want to be honest about how shaky this is though. This comes from test-tube and animal research, not real-world cases in people, and it's rated as a minor, possible concern. For most folks taking sildenafil for ED or pulmonary hypertension, occasional melatonin for sleep is unlikely to cause trouble. Still, it never hurts to run it by your pharmacist or doctor first.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
