Why this applies to Aliskiren

Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group Melatonin interacts with.

Theoretically, taking melatonin with antihypertensive drugs might increase the risk of hypotension or hypertension.
Some clinical research suggests that taking melatonin decreases blood pressure in healthy adults. Also, melatonin seems to lower systolic and diastolic blood pressure in individuals with high blood pressure at nighttime or untreated essential hypertension. However, melatonin seems to worsen blood pressure in patients who are taking antihypertensive medications. Immediate-release melatonin 5 mg at night in combination with nifedipine GITS (Procardia XL) increases systolic blood pressure an average of 6.5 mmHg, diastolic blood pressure by an average of 4.9 mmHg, and heart rate by 3.9 bpm. Also, results from animal research suggest that melatonin reduces the effectiveness of certain antihypertensive drugs, including methoxamine and clonidine.

Here's the short version: melatonin and your blood pressure medicine aliskiren (Tekturna) can pull in different directions. On its own, melatonin tends to nudge blood pressure down a little at night. But when people take it alongside a blood pressure drug, some research has actually found the opposite, with blood pressure and heart rate creeping up rather than down. That could make aliskiren feel less effective at keeping your numbers in range.

This is mostly a theoretical and modest concern, not a dangerous combination, and the clearest study used a different drug than yours. Still, if you take melatonin regularly, it's worth keeping an eye on your home blood pressure readings and mentioning it to your pharmacist or doctor before making any changes.

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version: your body partly clears aliskiren (Tekturna) using a liver enzyme called CYP3A4. Some lab studies suggest melatonin might slow that enzyme down a little. If that happened, a bit more aliskiren could linger in your system, which in theory might nudge its blood-pressure-lowering effect up. With aliskiren that could look like feeling lightheaded or dizzy, especially when standing up quickly.

But I want to keep this in perspective. This is only a theoretical concern based on test-tube research, not real patient cases, and it's rated as minor. Aliskiren is not a prodrug, so we're not flipping the direction here. For most people this won't cause problems, but it's worth a quick mention to your pharmacist or doctor before starting melatonin.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
