These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Melatonin interacts with whole categories of medication. Lorazepam falls into 2 of those categories, so it can interact with Melatonin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CNS DEPRESSANTS
1 of 2Why this applies to Lorazepam
Lorazepam falls under CNS DEPRESSANTS — the group Melatonin interacts with.
Melatonin has sedative effects. Theoretically, concomitant use of melatonin with alcohol, benzodiazepines, or other sedative drugs might cause additive sedation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: both melatonin and lorazepam nudge your brain toward calm and sleepiness. Lorazepam isn't a prodrug, so this isn't about one changing how the other is broken down. Instead, the concern is that their relaxing effects can simply stack on top of each other.
If you take them together, you might feel more drowsy, groggy, or lightheaded than usual, or feel a little unsteady on your feet. Honestly, this is mostly a theoretical concern based on how the two work, not strong proof of harm. Still, go easy until you know how the combination hits you, avoid driving if you feel foggy, and check with your pharmacist or doctor before pairing them, especially if you're older.

ANTICONVULSANTS
2 of 2Lorazepam also falls under ANTICONVULSANTS.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical angle for combining melatonin with lorazepam (Ativan). Both can make you sleepy, so the most likely thing you'd actually notice is added drowsiness, more grogginess, dizziness, or feeling unsteady on your feet, especially when you first pair them or get up at night. That's worth keeping in mind if you drive or are caring for others.
The interaction on file talks about melatonin possibly lowering the effect of seizure medicines and, in some people (mainly children with neurological conditions), nudging up seizure frequency. That signal is fairly theoretical and based on limited research. If you take lorazepam for seizures rather than anxiety, mention this to your pharmacist or doctor before adding melatonin. Please don't start or stop anything on your own.
What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.
- DAnecdotal evidence.

From our pharmacist’s overview on the Lorazepam page
“Lorazepam (Ativan) works by slowing brain activity, and that is exactly why this list deserves real caution. Of the 119 supplement and herbal ingredients on file, 78 are themselves nervous-system depressants, so adding them can deepen drowsiness or, at the extreme, slow breathing.”

About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
About Lorazepam
What Is Lorazepam Used For?
Brand name: Ativan
Lorazepam tablets (Ativan and generic) and the oral solution are used to manage anxiety disorders or for short-term relief of anxiety symptoms, including anxiety that comes with depressive symptoms. Loreev XR, an extended-release capsule, is specifically indicated for adults with anxiety disorders who are already stable on lorazepam tablets taken three times daily.
Ativan and generic lorazepam injections are used in two additional situations: treating status epilepticus (a medical emergency involving prolonged or repeated seizures) and as a pre-surgery medication in adults to produce sedation, ease anxiety, and reduce memory of events on the day of surgery.

Lorazepam Is Linked to Lower Levels of One Nutrient
Natural Medicines records Lorazepam as a medicine that can lower these. This is not an interaction with Melatonin — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
