Here's the thinking behind this one. Melatonin can give your immune system a little nudge to be more active. Lenalidomide (Revlimid) actually works in a more complex way, but it does have immune-related and immune-suppressing effects in conditions like multiple myeloma and myelodysplastic syndrome. The theory is that melatonin's immune-stimulating signal could work against what your cancer therapy is trying to do.

I want to be honest with you: this concern is theoretical and comes from lower-quality evidence, so it may not amount to much in real life. Lenalidomide is not a prodrug, so this isn't about how it's broken down. Still, because this is serious cancer treatment, I'd loop in your oncology team or pharmacist before adding melatonin rather than starting it on your own.

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

What Is Lenalidomide Used For?

Brand name: Revlimid

Lenalidomide capsules (brand name Revlimid) are used to treat several blood cancers and related conditions in adults. These include multiple myeloma (MM) — taken in combination with dexamethasone, or as maintenance therapy after a stem cell transplant — and transfusion-dependent anemia caused by low- or intermediate-1-risk myelodysplastic syndromes (MDS) with a specific chromosome change called deletion 5q.

Lenalidomide capsules are also used for mantle cell lymphoma (MCL) that has come back or stopped responding after at least two prior treatments (one of which must have included bortezomib), and for previously treated follicular lymphoma (FL) or marginal zone lymphoma (MZL) — both in combination with a rituximab product. It is not approved for chronic lymphocytic leukemia (CLL) outside of clinical trials.
