Why this applies to Sildenafil

Sildenafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cranberry interacts with.

Theoretically, cranberry might increase the levels and adverse effects of CYP3A4 substrates.
A case of upper back pain, rhabdomyolysis, and abnormal liver function has been reported for a patient taking atorvastatin, a CYP3A4 substrate, in combination with cranberry juice 16 ounces daily for 2 weeks. Creatinine kinase and liver enzymes normalized within 2 weeks of stopping cranberry juice. Also, animal research suggests that cranberry juice, administered intraduodenally 30 minutes prior to nifedipine, a CYP3A4 substrate, inhibits nifedipine metabolism and increases the area under the concentration-time curve by 1.6-fold compared to control.

Here's the gist with cranberry and sildenafil. Your body relies on a liver enzyme called CYP3A4 to break sildenafil down. The thought is that cranberry might slow that enzyme a little, which could let sildenafil hang around longer than usual. If that happens, you might notice more of its typical effects, like headache, facial flushing, a stuffy nose, indigestion, or feeling lightheaded as your blood pressure dips.

That said, I want to keep this in perspective. The evidence here is very thin, mostly theoretical plus a stray case report involving a different drug. Occasional cranberry juice or a UTI supplement is unlikely to cause real trouble for most people. Still, if you take sildenafil daily for lung pressure (Revatio) or notice odd symptoms, run it by your pharmacist or doctor first.

Sildenafil also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, cranberry might increase the levels and adverse effects of CYP2C9 substrates. However, research is conflicting.
There is contradictory evidence about the effect of cranberry on CYP2C9 enzymes. In vitro evidence suggests that flavonoids in cranberry inhibit CYP2C9 enzymes. However, clinical research shows that cranberry juice does not significantly affect the levels, metabolism, or elimination of the CYP2C9 substrates flurbiprofen or diclofenac. Also, in patients stabilized on warfarin, drinking cranberry juice 250 mL daily for 7 days does not significantly increase the anticoagulant activity of warfarin, a CYP2C9 substrate. Additional pharmacokinetic research shows that cranberry juice does not increase peak plasma concentrations or area under the concentration-time curve of warfarin.

Good news here: this is a very minor concern. The idea is that compounds in cranberry might slow down a liver enzyme called CYP2C9, which helps clear sildenafil (Viagra or Revatio) from your body. In theory, if that enzyme worked slower, sildenafil could build up a bit and you might notice more of its usual effects, like headache, facial flushing, a stuffy nose, or lightheadedness from lowered blood pressure.

In real life, though, this mostly hasn't panned out. Studies on cranberry juice with other CYP2C9 drugs, including warfarin, found no meaningful change. So for most people, normal cranberry intake is unlikely to cause a problem. If you take sildenafil regularly and use concentrated cranberry supplements, just run it by your pharmacist to be safe.

What Is Sildenafil Used For?

Brand name: Revatio

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
