Jiaogulan Drug Interactions: The Bottom Line

From the HelloPharmacist Editorial Team · Updated July 2026

Based on the available evidence, the overall risk of a clinically significant Jiaogulan drug interaction appears low for most people, though a few medication groups deserve real attention. None of the listed interactions are rated major, and none have been clearly confirmed in well-controlled human studies.

Interactions deserving the most attention

The best-supported concern involves diabetes medications, because clinical research shows jiaogulan can lower blood sugar, so combining them could add to that effect. Jiaogulan may also stimulate the immune system, which could work against immunosuppressants, a real worry for transplant patients whose drug effects are closely monitored. Lab research also hints at antiplatelet activity, so people on blood thinners should be aware of a theoretical bleeding risk, though this has not been shown in humans.

What the rest of the list means

Jiaogulan appears alongside hundreds of medications, but that long list reflects broad drug categories, not proven problems with each one. Most of these interactions are theoretical, based on lab or animal findings about how jiaogulan might affect blood sugar, clotting, or immune activity, and they have not been confirmed to cause meaningful changes in people.

What Severity, Likelihood & Evidence Mean

Severity — How Serious It Can Be

  • Major. Clinically significant; generally best avoided, or used only under direct professional supervision.
  • Moderate. May need monitoring, a dose adjustment, or separating the times you take each one.
  • Minor. Generally not clinically significant, but still worth noting and mentioning to your pharmacist.
  • No known interaction. Checked against our sources with nothing documented — not the same as proven safety.

Likelihood — How Well It’s Documented

  • Likely. Well-controlled human studies have demonstrated the likely existence of this interaction
  • Probable. Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • Possible. Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists
  • Unlikely. Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

Where This Data Comes From

  • Interaction records are evidence-graded and sourced from the Natural Medicines database (TRC Healthcare), the same reference used by pharmacists and hospitals.

Overview

Jiaogulan (Gynostemma pentaphyllum) is a climbing vine that grows in warm, humid parts of southern China and other regions of Asia. It is sometimes called "Southern Ginseng" or the "Immortality Herb" because some of its plant compounds are similar to those found in ginseng, and because it has a long history of traditional use.

In traditional Chinese medicine, jiaogulan has been used as a daily tea and a general tonic. Today people take it as a tea, capsule, tablet, or liquid extract for energy, stress, and to support the heart and metabolism. It is often marketed as an adaptogen, a term used for herbs thought to help the body cope with stress.

How it works

Jiaogulan contains many plant compounds called gypenosides, which are a type of saponin. Some of these are chemically similar to the active compounds (ginsenosides) found in ginseng. The plant also contains flavonoids and other antioxidants.

Laboratory and animal research suggests these compounds may act as antioxidants, help cells respond to stress, support healthy blood sugar handling, and affect cholesterol and blood vessel function. Some studies suggest it may activate an enzyme called AMPK that is involved in energy and metabolism.

It is important to remember that most of these mechanisms come from test-tube and animal studies. How well they apply to people taking the herb is not fully known.

Safety & precautions

Jiaogulan is generally considered well tolerated when used as a tea or supplement for short periods, but its long-term safety has not been carefully studied.

People with diabetes should be cautious, since jiaogulan may lower blood sugar and could add to the effect of medications. If you have low blood pressure, are scheduled for surgery, or have a bleeding disorder, talk with your doctor first, as some sources suggest it may affect blood pressure or clotting.

Pregnancy and breastfeeding: Avoid jiaogulan if you are pregnant or breastfeeding. There is not enough safety information, and some animal data raise concerns about effects on a developing baby. The cautious choice is to skip it.

Always tell your pharmacist or doctor about any supplements you take, especially if you have a chronic health condition.

Side effects

Jiaogulan is usually well tolerated. The most commonly reported side effects are mild and involve the stomach, such as nausea or increased bowel movements.

Because it may lower blood sugar, some people could feel symptoms of low blood sugar, such as shakiness, sweating, or dizziness, especially if they also take diabetes medicine. Serious side effects appear to be rare, but because long-term use has not been well studied, it is wise to start low and watch how you feel.

Dosing

There is no official, standardized dose of jiaogulan. The right amount can vary depending on the product, its strength, and whether it is a tea, capsule, or extract.

The safest approach is to follow the directions on the product label and not to exceed the recommended amount. Because product quality and strength can differ between brands, choose products from reputable companies, ideally ones with third-party testing.

Before starting jiaogulan, check with your pharmacist or doctor, especially if you take medications or have diabetes, blood pressure, or other health conditions.

Jiaogulan: Common Questions

What is Jiaogulan used for, and does it work?
People use Jiaogulan for many reasons, but the evidence varies by use. According to the Natural Medicines database, there is at least some clinical evidence supporting Jiaogulan for Diabetes. For most other uses it has been studied for, the evidence is currently insufficient to rate. See the graded list on this page, and always confirm with your pharmacist or doctor.
Does Jiaogulan interact with prescription medications?
Yes. We list 327 medications with a known interaction with Jiaogulan. Use the checker above to see how Jiaogulan interacts with a specific drug.
How are Jiaogulan interactions rated?
Each interaction is graded major, moderate, or minor based on how clinically significant it is and the strength of the evidence behind it. Major interactions are the most serious and are best avoided, or used only under close professional supervision.
Is it safe to take Jiaogulan with my medications?
It depends on the specific medication. Some combinations are fine, while others call for monitoring, timing changes, or should be avoided. Check your exact drug above and confirm with your pharmacist or doctor before starting, stopping, or changing anything.
Where does this Jiaogulan interaction information come from?
Our interaction data is built on the Natural Medicines database — an evidence-graded reference for vitamins, herbs, and supplements — and is reviewed by licensed HelloPharmacist pharmacists, who may add clinical context.
What is Jiaogulan used for?
It is traditionally used as an adaptogen for energy and stress, and for supporting blood sugar, cholesterol, and blood pressure. Human evidence is strongest for blood sugar and metabolism, but it is still limited overall.
Is Jiaogulan safe to take?
It is generally well tolerated for short-term use, with mild stomach upset being the most common complaint. Long-term safety is not well studied, so it is best to check with your pharmacist or doctor first.
Can I take Jiaogulan while pregnant or breastfeeding?
No, it is best to avoid it. There is not enough safety information, so the cautious choice is to skip Jiaogulan during pregnancy and breastfeeding.
Is Jiaogulan the same as ginseng?
No. It is sometimes called "Southern Ginseng" because some of its compounds resemble those in ginseng, but it is a different plant from a different family.
