Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Sarilumab
Sarilumab falls under IMMUNOSUPPRESSANTS — the group Jiaogulan interacts with.
Clinical and animal studies suggest that jiaogulan can stimulate the immune system.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Kevzara (sarilumab). It's an injected biologic that calms down an overactive immune system to ease rheumatoid arthritis pain, swelling, and joint damage. Jiaogulan, on the other hand, has some early lab and animal research suggesting it may stimulate the immune system. So in theory, the two could work against each other, and jiaogulan might chip away at how well sarilumab controls your RA.
I want to be honest that this is a theoretical concern based on how jiaogulan behaves in studies, not something proven in people taking sarilumab. Still, since flaring arthritis is no fun, I'd loop in your rheumatologist or pharmacist before adding jiaogulan so they can keep an eye on your joints.

From our pharmacist’s overview on the Sarilumab page
“Sarilumab (Kevzara) works by quieting an overactive immune system in rheumatoid arthritis, and most of the supplements on this page push the other way. Immune stimulants such as ashwagandha, astragalus, and American ginseng could in theory blunt the drug's benefit and let joint symptoms creep back.”

About Jiaogulan
What is Jiaogulan?
- Fatigue and stress (adaptogen)
- Blood sugar support
- High blood pressure
- High cholesterol
- Immune and general wellness
Jiaogulan is a climbing vine used in traditional Chinese medicine as an adaptogen and for heart, blood sugar, and cholesterol support. Early human and animal studies are promising for some uses like metabolism and blood pressure, but the overall evidence is still limited. Talk with your pharmacist or doctor before starting it, especially if you have a health condition or take medication.
About Sarilumab Injection
What Is Sarilumab Injection Used For?
Brand name: Kevzara
Kevzara (sarilumab) injection is approved for three conditions. In adults, it treats moderately to severely active rheumatoid arthritis (RA) when at least one other disease-modifying drug hasn't worked well enough or caused intolerance. It also treats polymyalgia rheumatica (PMR) in adults who haven't responded adequately to corticosteroids or who can't tolerate tapering them.
Kevzara is also approved for polyarticular juvenile idiopathic arthritis (pJIA) — a form of childhood arthritis — but only in patients who weigh 63 kg (about 139 lbs) or more. It is not approved for pediatric patients below that weight due to the lack of an appropriate dosage form.
