Sarilumab (Kevzara) works by quieting an overactive immune system in rheumatoid arthritis, and most of the supplements on this page push the other way. Immune stimulants such as ashwagandha, astragalus, and American ginseng could in theory blunt the drug's benefit and let joint symptoms creep back. Nothing here calls for blanket avoidance, but each combination is worth knowing about and watching for signs the medicine is losing ground.

The smaller cluster works differently. Sarilumab is grouped with drugs that boost a liver enzyme called CYP3A4, which matters for herbs containing pyrrolizidine alkaloids (boneset and borage among them), since faster processing can create more liver-toxic byproducts. The same effect can lower CBD levels.
