Why this applies to Sarilumab
Sarilumab falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Groundsel interacts with.
Pyrrolizidine alkaloids, constituents of groundsel, are converted to toxic metabolites by CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
First things first: I really wouldn't take groundsel at all. It contains pyrrolizidine alkaloids, which are known to harm the liver, and it isn't proven to help anything. The concern here is a bit indirect. Groundsel's toxic compounds get turned into their most damaging form by a liver enzyme called CYP3A4, and certain medicines can speed that enzyme up, which could mean more of those harmful byproducts and more liver stress.
Now, Sarilumab (Kevzara) is a biologic antibody, not a typical pill broken down by CYP3A4, so this enzyme effect doesn't really fit it the way it fits oral drugs. This warning is theoretical and based only on lab data. Even so, since RA biologics already get monitored for liver effects, the safest move is simple: skip the groundsel and check with your pharmacist or doctor.

Groundsel is a common weed traditionally used as a folk remedy, but it contains pyrrolizidine alkaloids that can cause serious liver damage. There is no reliable evidence that it works for any condition, and most health authorities consider it unsafe to take internally. We do not recommend using it.
About Sarilumab Injection
What Is Sarilumab Injection Used For?
Brand name: Kevzara
Sarilumab Injection (Kevzara) treats certain inflammatory conditions. In adults, it is used for moderately to severely active rheumatoid arthritis after one or more disease-modifying antirheumatic drugs (DMARDs) did not work well enough or were not tolerated.
Kevzara is also used in adults with polymyalgia rheumatica when corticosteroids did not work well enough or a steroid taper could not be tolerated. It is also used for active polyarticular juvenile idiopathic arthritis in patients who weigh 63 kg or more.
