Groundsel and Luliconazole

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Why this applies to Luliconazole

Luliconazole falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Groundsel interacts with.

Theoretically, taking CYP3A4 inducers with groundsel might increase the risk of pyrrolizidine alkaloid toxicity, such as hepatotoxicity.
Pyrrolizidine alkaloids, constituents of groundsel, are converted to toxic metabolites by CYP3A4.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news for this particular pairing: Luzu (luliconazole) is a cream you rub onto your skin for athlete's foot, jock itch, or ringworm. Very little of it gets into your bloodstream, so it isn't really acting like a body-wide CYP3A4 booster the way an oral drug might. The concern on file is theoretical and comes only from lab or animal data, where CYP3A4 can turn groundsel's pyrrolizidine alkaloids into liver-damaging byproducts.

My honest take? The bigger issue is groundsel itself. It can seriously harm the liver and we don't recommend taking it internally at all. A topical antifungal isn't likely to meaningfully change that risk. If you're using any groundsel product, please run it by your pharmacist or doctor first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Groundsel

What is Groundsel?

  • Menstrual complaints
  • Digestive upset
  • Minor wounds (topical)
  • Sore gums or mouth
  • Mild aches

Groundsel is a common weed traditionally used as a folk remedy, but it contains pyrrolizidine alkaloids that can cause serious liver damage. There is no reliable evidence that it works for any condition, and most health authorities consider it unsafe to take internally. We do not recommend using it.
