First things first: I'd steer clear of groundsel entirely. It contains pyrrolizidine alkaloids that can seriously harm the liver, and there's no good reason to take it. But to your actual question about Fosaprepitant (Emend Injection), the concern here runs in a slightly unexpected direction. The liver enzyme CYP3A4 is what turns groundsel's alkaloids into their toxic forms. Anything that revs up that enzyme could, in theory, make groundsel more damaging to the liver, not less.

Now, fosaprepitant's effect on CYP3A4 is a bit mixed and short-lived, and this whole concern comes only from lab and animal data, not real patients. So I wouldn't lose sleep over the combination itself. The bigger issue is simply taking groundsel at all. Please run any supplement past your pharmacist or doctor before using it.
