Here's the connection worth knowing. Once adrafinil reaches your liver, it gets turned into modafinil, and your body relies on an enzyme called CYP3A4 to help clear that modafinil out. Fosaprepitant (Emend Injection), which is given to prevent nausea and vomiting from chemotherapy, can mildly slow down that same enzyme for a short while.

So in theory, modafinil from your adrafinil could hang around a little longer and feel stronger, meaning more jitteriness, trouble sleeping, headache, a faster heartbeat, or anxiety. I'll be honest with you though: this is based on how the drugs work on paper, not on real-world reports, so it's a possibility rather than a sure thing. Since fosaprepitant is given in a clinical setting, please mention any adrafinil use to your care team first.

Here's the gist for this pairing. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up a liver enzyme called CYP3A4. Fosaprepitant (Emend Injection) is handled by that same enzyme, so in theory a faster enzyme could clear it a bit quicker and leave you with slightly less of the drug on board.

Fosaprepitant is given to help prevent nausea and vomiting from chemotherapy, so the practical worry is that it might not work quite as well, meaning more queasiness or vomiting than expected. I want to be honest though: this is a theoretical concern based on how the drugs behave, not proven cases, and modafinil is only a weak enzyme booster. Still, please run it by your oncology team or pharmacist before combining them.
