Groundsel + Rifampin

Rifampin brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor.

Moderate Interaction
The bottom line Moderate

Groundsel has a moderate interaction with Rifampin. Theoretically, taking CYP3A4 inducers with groundsel might increase the risk of pyrrolizidine alkaloid toxicity, such as hepatotoxicity.

Groundsel and Rifampin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Why this applies to Rifampin

Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Groundsel interacts with.

Theoretically, taking CYP3A4 inducers with groundsel might increase the risk of pyrrolizidine alkaloid toxicity, such as hepatotoxicity.
Pyrrolizidine alkaloids, constituents of groundsel, are converted to toxic metabolites by CYP3A4.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: I really don't recommend taking groundsel at all. It contains pyrrolizidine alkaloids that can seriously hurt your liver, and there's no good proof it helps anything. Here's the wrinkle with rifampin specifically. Rifampin is a powerful inducer, meaning it revs up a liver enzyme called CYP3A4. That same enzyme is what turns groundsel's alkaloids into their toxic, liver-damaging form.

So in theory, pairing the two could ramp up that conversion and raise your risk of liver injury. Rifampin already stresses the liver on its own (it can cause yellowing skin or eyes, dark urine, and belly pain), so I'd hate to add another liver burden. To be fair, this concern comes only from lab and animal data, not human studies, so it's theoretical. Still, skip the groundsel and check with your doctor or pharmacist.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Groundsel

What is Groundsel?

  • Menstrual complaints
  • Digestive upset
  • Minor wounds (topical)
  • Sore gums or mouth
  • Mild aches

Groundsel is a common weed traditionally used as a folk remedy, but it contains pyrrolizidine alkaloids that can cause serious liver damage. There is no reliable evidence that it works for any condition, and most health authorities consider it unsafe to take internally. We do not recommend using it.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.

It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.

Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
