Why this applies to Sarilumab

Sarilumab falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's a wrinkle that surprises people: this concern isn't about borage changing how your Kevzara (sarilumab) works. It's the other way around. Sarilumab is an injected antibody, not a pill broken down by liver enzymes, so borage won't really raise or lower its effect. But because RA inflammation can actually slow certain liver enzymes, calming that inflammation with sarilumab may speed up an enzyme called CYP3A4.

Why does that matter? Some parts of the borage plant (leaf, flower, seed) carry liver-toxic compounds called pyrrolizidine alkaloids, and a faster CYP3A4 could, in theory, make more of the toxic byproduct. This is a lab-based, theoretical worry, not something proven in people. The simple fix: only use products clearly labeled PA-free, and check with your pharmacist or doctor first.

About Sarilumab Injection

What Is Sarilumab Injection Used For?

Brand name: Kevzara

Kevzara (sarilumab) injection is approved for three conditions. In adults, it treats moderately to severely active rheumatoid arthritis (RA) when at least one other disease-modifying drug hasn't worked well enough or caused intolerance. It also treats polymyalgia rheumatica (PMR) in adults who haven't responded adequately to corticosteroids or who can't tolerate tapering them.

Kevzara is also approved for polyarticular juvenile idiopathic arthritis (pJIA) — a form of childhood arthritis — but only in patients who weigh 63 kg (about 139 lbs) or more. It is not approved for pediatric patients below that weight due to the lack of an appropriate dosage form.
