Phenytoin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the angle that matters with this pairing. Phenytoin doesn't get harmed by borage directly. Instead, phenytoin is a known CYP3A4 inducer, meaning it revs up that liver enzyme. The concern is that borage leaf, flower, or seed products can carry natural compounds called pyrrolizidine alkaloids (PAs), and CYP3A4 is what turns those PAs into liver-toxic byproducts. So with phenytoin speeding up that enzyme, your body could theoretically make more of the harmful metabolites, raising the risk of liver injury.

Keep in mind this is theoretical, based on lab data only, and properly made borage seed oil usually has little to no PAs. If you do use borage, stick to products clearly labeled "PA-free," and check with your pharmacist or doctor first.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
