Carbamazepine falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the key with carbamazepine and borage: carbamazepine is a strong booster (an inducer) of a liver enzyme called CYP3A4. That same enzyme can turn the pyrrolizidine alkaloids (PAs) found in some borage plant parts into harmful, liver-toxic byproducts. So the worry isn't that carbamazepine stops working. It's that revving up CYP3A4 could create more of those toxic PA metabolites if your borage product happens to contain PAs.

The good news is this is theoretical, based only on lab data, and borage seed oil usually has little to no PAs. The simplest safeguard is to use only products clearly labeled PA-free. Since carbamazepine itself is a tricky drug to balance, please run any borage product past your pharmacist or doctor first.

About Carbamazepine

What Is Carbamazepine Used For?

Brand names: Tegretol, Tegretol XR, Equetro, Carbatrol, Carnexiv

Carbamazepine is used to treat certain types of seizures — including partial seizures, generalized tonic-clonic (grand mal) seizures, and mixed seizure patterns. It works best for those specific seizure types; it does not appear to control absence (petit mal) seizures.

It is also prescribed to relieve the intense facial pain of trigeminal neuralgia — a nerve condition that causes sudden, severe pain along the face. It is not a general painkiller and shouldn't be used for ordinary aches and pains.
