Cenobamate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the thing to understand about this pairing: cenobamate (Xcopri) speeds up an enzyme in your liver called CYP3A4. That same enzyme is what turns the natural toxins in certain borage plant parts, called pyrrolizidine alkaloids (PAs), into harmful byproducts that can stress your liver. So in theory, cenobamate could ramp up that conversion and raise your risk of liver irritation if your borage product contains PAs.

The good news is this is theoretical, based only on lab studies, not real patients. And borage seed oil usually has little to no PAs. The safe move is simple: only use products clearly labeled PA-free, and check with your pharmacist or doctor first, since your seizure medicine is too important to mess with.
