Oxcarbazepine falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the angle that matters with this pairing. Oxcarbazepine is one of the seizure medicines that can speed up the CYP3A4 enzyme in your liver. Normally that just affects how other drugs are cleared. But borage leaf, flower, and seed can contain pyrrolizidine alkaloids (PAs), natural compounds that CYP3A4 turns into liver-damaging byproducts. The worry is that revving up that enzyme could create more of those toxic byproducts.

The good news: this is a theoretical concern based on lab data, not real patient reports, and quality borage seed oil contains little to no PAs. The simple safeguard is to use only products clearly labeled 'PA-free.' Since oxcarbazepine itself needs careful, steady management, check with your pharmacist or doctor before adding any borage product.

What Is Oxcarbazepine Used For?

Brand names: Trileptal, Oxtellar XR

Oxcarbazepine treats partial-onset seizures (seizures that start in one area of the brain). It can be used alone (monotherapy) or alongside other seizure medicines (adjunctive therapy).

For adults, oxcarbazepine tablets, film-coated tablets, and oral suspension are approved for both monotherapy and adjunctive therapy. Oxcarbazepine extended-release tablets are approved for partial-onset seizures in patients 6 years and older. In children, Trileptal and oxcarbazepine oral suspension are approved as monotherapy in children aged 4–16 and as adjunctive therapy in children aged 2–16.
