Oritavancin Diphosphate falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Borage interacts with.

Theoretically, taking borage with drugs that induce CYP3A4 might increase levels of pyrrolizidine alkaloid (PA) toxic metabolites.
Although borage seed oil contains little to no PAs, some borage plant parts, such as the leaf, flower, and seed, can contain hepatotoxic PAs. Hepatotoxic PAs are substrates of CYP3A4, which converts these chemicals into toxic metabolites. Tell patients to avoid borage preparations that are not certified and labeled as hepatotoxic PA-free.

Here's the thing to understand: this interaction isn't really about the antibiotic harming you or losing its punch. Oritavancin (Orbactiv, Kimyrsa) happens to act as a mild CYP3A4 inducer, meaning it can speed up that liver enzyme. The concern is what that does to borage. Certain borage plant parts (the leaf, flower, and seed) contain pyrrolizidine alkaloids, and CYP3A4 is the enzyme that turns those compounds into liver-toxic byproducts. So in theory, the antibiotic could ramp up production of those harmful metabolites.

I'll be honest, this is based on lab and animal data, not real patient reports, so don't panic. The simplest safeguard is to only use borage products clearly labeled PA-free, like quality borage seed oil. Run it by your pharmacist or doctor first.

What Is Oritavancin Injection Used For?

Oritavancin injection — available as Orbactiv and Kimyrsa — is used to treat acute bacterial skin and skin structure infections (ABSSSI) in adults. These are serious skin infections caused by susceptible gram-positive bacteria, including Staphylococcus aureus (including MRSA), several Streptococcus species, and vancomycin-susceptible Enterococcus faecalis.

This antibiotic should only be used when a bacterial infection is confirmed or strongly suspected. Using it unnecessarily can contribute to antibiotic resistance.
