Acenocoumarol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Borage interacts with.

Theoretically, borage seed oil may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In healthy individuals, borage seed oil supplementation does not seem to affect platelet aggregation. However, gamma-linolenic acid, a constituent of borage seed oil, seems to decrease platelet aggregation by 45% and increase the risk of bleeding by 40% in animal and clinical research.

Here's the gist: acenocoumarol (Sintrom) is a blood thinner, and it's what we call a narrow-therapeutic-index drug, meaning the gap between too little and too much is small. Borage seed oil contains GLA, which in some research made platelets a little less sticky and nudged bleeding risk up. Stack that mild blood-thinning effect on top of Sintrom and you could tip toward too much thinning.

What that looks like in real life: easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool. The evidence here is on the weaker side and mostly theoretical, so don't panic. But because Sintrom is dose-sensitive, please loop in your pharmacist or doctor before adding borage, and pick a 'PA-free' product if you do.
