None of the interactions are rated major, and all of them rest on the same theoretical concern rather than documented effects in people.
The one group that stands out is CYP3A4 inducers, medications that speed up a liver enzyme the body uses to process many drugs. Hound's Tongue naturally contains pyrrolizidine alkaloids, compounds that this enzyme can convert into byproducts harmful to the liver. In theory, taking an enzyme-inducing medication could increase that conversion and raise the chance of liver injury. This has been suggested by how these compounds are processed, not confirmed in human studies, but because the potential harm involves the liver, we take it seriously.
The long list of interacting medications reflects a single shared mechanism, not dozens of separate proven problems. Every entry traces back to the same theoretical enzyme effect, and none has been shown to cause meaningful harm in people so far. Still, the plant's natural alkaloids are the real reason for caution here, more than any one drug pairing.

Cytochrome P450 3A4 (Cyp3A4) Inducers

Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Overview
Hound's Tongue (Cynoglossum officinale) is a flowering plant in the borage family. It gets its name from its long, soft, tongue-shaped leaves. The plant has small reddish-purple flowers and produces burr-like seeds that stick to clothing and animal fur.
It is native to Europe and parts of Asia and now grows as a weed in many areas of North America, often along roadsides, pastures, and disturbed soil. In some places it is considered an invasive plant.
Historically, folk healers used the leaves and roots in poultices, ointments, and teas. Most traditional use was for skin problems and minor coughs. Today it is rarely found in mainstream supplements, and modern herbalists are very cautious because of its toxic plant compounds.
Uses & effectiveness
Hound's Tongue has a long history in folk medicine, but there is very little scientific evidence to support any of its uses. Most of what we know comes from old traditional practices rather than well-designed human studies.
People have traditionally used it on the skin for minor wounds, burns, bruises, and hemorrhoids, and have taken it internally for coughs, sore throat, and diarrhea. None of these uses are backed by strong clinical research.
In short, the evidence for Hound's Tongue is insufficient for every common use. Because of safety concerns, the lack of proof matters even more here.
How it works
Hound's Tongue contains compounds such as mucilage, tannins, and pyrrolizidine alkaloids. The mucilage is a slippery, gel-like substance that may soothe irritated tissue, which could explain its traditional use on the skin and throat. Tannins have a drying, astringent effect that folk healers linked to wound care and diarrhea.
The pyrrolizidine alkaloids are the most important compounds from a safety view. These chemicals are known to be toxic to the liver and can build up with repeated use.
It is important to know that these ideas are based mostly on the plant's chemistry and on laboratory or animal research, not on solid human studies showing it treats any condition.
Safety & precautions
The biggest concern with Hound's Tongue is its pyrrolizidine alkaloids. These compounds are known to damage the liver and have been linked to a serious condition where blood flow through the liver becomes blocked. They may also be cancer-causing over time. For this reason, taking Hound's Tongue by mouth is generally discouraged.
Because the alkaloids are absorbed through the skin, even applying it topically over broken skin or for long periods may not be safe. People with liver problems should be especially careful and should avoid it.
Pregnancy and breastfeeding: Avoid Hound's Tongue completely. The toxic alkaloids may harm the liver, can cross to the developing baby, and may pass into breast milk. There is no safe established use during these times.

Side effects
Reliable data on side effects in people is limited, but the known risks are serious. The pyrrolizidine alkaloids can cause liver damage, which may show up as nausea, abdominal pain, fatigue, yellowing of the skin or eyes (jaundice), or swelling in the belly.
Liver injury from these alkaloids can be severe and sometimes permanent, and it may build up quietly before symptoms appear. Skin contact could cause irritation in some people.
Because of these risks, stop use and seek medical care right away if you notice signs of liver trouble.

The pyrrolizidine alkaloid (PA) constituents of hound's tongue are hepatotoxic. Chronic exposure to other plants containing hepatotoxic PA constituents is associated with veno-occlusive disease (VOD). Subacute VOD causes vague symptoms with persistent liver enlargement. Symptoms of acute VOD include epigastric pain, vomiting and diarrhea, and ascites formation within several days. Enlargement and induration of the liver occurs within a few weeks.

Dosing
There is no established safe or standardized dose for Hound's Tongue. Because of its toxic alkaloids, no reliable dosing guidelines exist, and health authorities generally advise against internal use.
If you encounter a product that contains it, follow the product label exactly and, more importantly, talk with a pharmacist or doctor first. In most cases, safer and better-studied alternatives are available for the conditions people use this plant for.
