Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Hound's Tongue interacts with.

Theoretically, CYP3A4 inducers can increase the toxicity of the pyrrolizidine alkaloid (PA) constituents of hound's tongue.
Hepatotoxic PAs are substrates of cytochrome P450 3A4 (CYP3A4). Drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Here's the key thing with this pairing: it's not your cancer medicine that gets thrown off, it's the Hound's Tongue that becomes more dangerous. Lorlatinib (Lorbrena) speeds up an enzyme in your liver called CYP3A4. The pyrrolizidine alkaloids in Hound's Tongue are turned into liver-damaging byproducts by that very same enzyme. So in theory, taking lorlatinib could ramp up how fast those harmful compounds form, putting extra strain on your liver.

I'll be honest, this concern comes from lab work, not real patient studies, so it's theoretical. But Hound's Tongue is already known to harm the liver on its own, and lorlatinib can affect liver enzymes too. That's a stack I'd rather you avoid. Please don't change your lorlatinib, and check with your oncology team or pharmacist before using Hound's Tongue in any form.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
