Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the basic idea with these two. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP3A4. Your cancer medicine, Lorlatinib (Lorbrena), is broken down by that same enzyme. So in theory, adrafinil could make your body clear lorlatinib a little faster, which might lower its blood levels and weaken how well it fights your cancer.

I want to be honest about the strength of this: it's theoretical, based on how the drugs work rather than real-world cases, and modafinil is only a weak booster of that enzyme. Still, because lorlatinib is treating something this serious, you don't want anything chipping away at it. Please don't change either one on your own. Check with your oncologist or pharmacist first.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
