Sarilumab falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Evodia interacts with.

Theoretically, taking CYP3A4 inducers might decrease the levels and clinical effects of evodia.
Animal research shows that concomitant administration of dexamethasone, a known CYP3A4 inducer, with the alkaloid constituents of evodia significantly reduces the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the interesting twist with this pairing: the concern isn't that Evodia changes how Sarilumab (Kevzara) works. It's actually the other way around. Sarilumab can act like a CYP3A4 inducer, meaning it can speed up the liver enzyme that breaks down Evodia's active compounds. In theory, that could clear the Evodia from your body faster and make it less effective for whatever you're hoping it will do.

One thing to keep in mind: this is based only on lab and animal research, so it's fairly theoretical and not something I'd lose sleep over. Sarilumab itself shouldn't become stronger or weaker because of Evodia. Still, since herbal supplements aren't well studied alongside RA biologics, please run it by your pharmacist or rheumatologist before combining them.

Human evidence for these uses is very limited, and it is mostly studied in laboratory and animal research. About Sarilumab Injection

What Is Sarilumab Injection Used For?

Brand name: Kevzara

Kevzara (sarilumab) injection is approved for three conditions. In adults, it treats moderately to severely active rheumatoid arthritis (RA) when at least one other disease-modifying drug hasn't worked well enough or caused intolerance. It also treats polymyalgia rheumatica (PMR) in adults who haven't responded adequately to corticosteroids or who can't tolerate tapering them.

Kevzara is also approved for polyarticular juvenile idiopathic arthritis (pJIA) — a form of childhood arthritis — but only in patients who weigh 63 kg (about 139 lbs) or more. It is not approved for pediatric patients below that weight due to the lack of an appropriate dosage form.
