Sarilumab falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the thing about combining Gravel Root with Sarilumab (Kevzara): this herb may contain pyrrolizidine alkaloids, plant compounds that can stress the liver. They get changed into toxic byproducts by a liver enzyme called CYP3A4. The worry on file is that when a drug speeds up (induces) that enzyme, more of the herb gets turned into those harmful byproducts, which could mean more liver injury.

What's interesting is that Sarilumab actually works a little differently. By calming the inflammation of RA, it can restore normal CYP3A4 activity that inflammation had been suppressing. Either way, the real concern here is the herb's potential to harm your liver, not Sarilumab failing to work. This flag is theoretical and based only on lab data, so don't panic, but please run it by your doctor or pharmacist before mixing the two.

Gravel Root is a traditional North American herb long used by herbalists for kidney stones and urinary problems, but there is very little modern scientific evidence to support these uses. Because it contains compounds that may harm the liver and its safety is poorly studied, it should be used with caution, if at all, and avoided in pregnancy and breastfeeding.

What Is Sarilumab Injection Used For?

Brand name: Kevzara

Kevzara (sarilumab) injection is approved for three conditions. In adults, it treats moderately to severely active rheumatoid arthritis (RA) when at least one other disease-modifying drug hasn't worked well enough or caused intolerance. It also treats polymyalgia rheumatica (PMR) in adults who haven't responded adequately to corticosteroids or who can't tolerate tapering them.

Kevzara is also approved for polyarticular juvenile idiopathic arthritis (pJIA) — a form of childhood arthritis — but only in patients who weigh 63 kg (about 139 lbs) or more. It is not approved for pediatric patients below that weight due to the lack of an appropriate dosage form.
