Mephenytoin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the thing to understand about this pairing. Gravel Root may contain pyrrolizidine alkaloids, plant compounds that can be hard on the liver. Mephenytoin (Mesantoin) is an older seizure medicine that, like its cousin phenytoin, can speed up a liver enzyme called CYP3A4. The worry is that revving up this enzyme could turn more of those Gravel Root compounds into toxic byproducts, potentially adding to liver stress.

I want to be honest about the strength of this: it's a theoretical concern based on lab and animal data, not real-world reports in people. Still, Gravel Root is poorly studied and already carries liver-safety questions on its own. I'd skip it here and check with your doctor or pharmacist before combining anything new with your seizure medicine.
