Primidone falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with this pairing, and it's a bit different from a typical drug interaction. Gravel Root contains pyrrolizidine alkaloids, plant compounds that can be hard on the liver. Your body uses an enzyme called CYP3A4 to process these compounds, and Primidone is known to speed that enzyme up. In theory, a faster enzyme could turn more of the herb's alkaloids into the toxic, liver-damaging forms. So the worry here is added stress on your liver, not a change in how well your seizure medicine works.

I'll be honest: this is based on lab and animal data only, so it's theoretical, not proven in people. Still, Gravel Root already has liver-safety question marks, and Primidone is a long-term seizure medicine you don't want to disrupt. Please check with your pharmacist or doctor before combining them.

What Is Primidone Used For?

Brand name: Mysoline

Primidone tablets — sold as Mysoline or under the generic name primidone — are used to control seizures in people with epilepsy. Both Mysoline and primidone tablets are approved for grand mal (generalized tonic-clonic), psychomotor, and focal (partial) epileptic seizures.

Primidone can be used alone or alongside other anti-seizure medicines. It may also help control grand mal seizures that haven't responded well to other anticonvulsant treatments.
