Zanubrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the situation with Gravel Root and Brukinsa (zanubrutinib). The real concern isn't about your cancer medicine becoming weaker or stronger. It's the other direction. Gravel Root may contain pyrrolizidine alkaloids, and your liver uses an enzyme called CYP3A4 to handle them. Zanubrutinib can nudge that same enzyme to work a little faster. In theory, that could mean more of Gravel Root's compounds get turned into substances that may stress the liver.

I want to be honest that this is theoretical, based on lab work, not real patient studies. Still, since zanubrutinib already gets monitored for liver effects, I'd skip an herb with shaky safety data and possible liver risk. Please check with your oncology team or pharmacist before adding it.

Gravel Root is a traditional North American herb long used by herbalists for kidney stones and urinary problems, but there is very little modern scientific evidence to support these uses. About Zanubrutinib

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with certain blood cancers. It treats mantle cell lymphoma (MCL) in patients who have had at least one prior therapy, Waldenström's macroglobulinemia (WM), relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 treatment, and chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Brukinsa is also used — in combination with another medicine called obinutuzumab — to treat relapsed or refractory follicular lymphoma (FL) in adults who have already received two or more lines of treatment. Several of these uses were granted accelerated FDA approval based on response rates, meaning confirmatory trials are still ongoing.
