Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for Brukinsa (zanubrutinib). Your liver uses an enzyme called CYP3A4 to handle this cancer medicine. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that enzyme. In theory, that could clear zanubrutinib out a little faster, which might lower how much active drug stays in your system and could mean a weaker effect against your lymphoma or leukemia.

I want to be honest about the strength of this: it's theoretical, based on how these drugs behave on paper, not on real patient studies. Still, with a cancer treatment you don't want to gamble on lost effectiveness. Please loop in your oncologist or pharmacist before combining these, since adrafinil also carries its own liver risks.

What Is Zanubrutinib Used For?

Brand name: Brukinsa

Brukinsa (zanubrutinib) is used to treat adult patients with several blood cancers. These include chronic lymphocytic leukemia (CLL), small lymphocytic lymphoma (SLL), Waldenström's macroglobulinemia (WM), mantle cell lymphoma (MCL) after at least one prior treatment, relapsed or refractory marginal zone lymphoma (MZL) after at least one prior anti-CD20 regimen, and relapsed or refractory follicular lymphoma (FL) in combination with obinutuzumab after two or more prior lines of treatment.

Some of these uses — MCL, MZL, and FL — are approved under accelerated approval, meaning they are based on tumor response rates rather than confirmed long-term survival benefit. Continued approval for those uses may depend on results from confirmatory trials.
