Fosphenytoin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Gravel Root interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the angle to focus on with Gravel Root and fosphenytoin (Cerebyx). Gravel Root may contain pyrrolizidine alkaloids, plant compounds that can be hard on the liver. Fosphenytoin (which your body turns into phenytoin) is a known CYP3A4 inducer, meaning it speeds up that liver enzyme. The concern is that a faster enzyme could convert more of those alkaloids into toxic byproducts, potentially adding to liver stress.

I want to be honest that this is theoretical and based only on lab and animal data, not human studies, so it's not a reason to panic. Still, given Gravel Root's poorly studied liver safety, I'd lean toward avoiding it while on fosphenytoin. Please check with your doctor or pharmacist before adding any herbal product.
