Rice Bran Arabinoxylan Compound and Sarilumab

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Sarilumab

Sarilumab falls under IMMUNOSUPPRESSANTS — the group Rice Bran Arabinoxylan Compound interacts with.

Theoretically, rice bran arabinoxylan compound might interfere with immunosuppressants by stimulating the immune system. In laboratory and human research, rice bran arabinoxylan compound has been shown to stimulate the activity of immune cells.
Some immunosuppressants include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic tension with this pairing. Sarilumab (Kevzara) works by quieting part of your immune system to calm the inflammation that drives rheumatoid arthritis. Rice Bran Arabinoxylan Compound is sold to do the opposite, to rev up immune cell activity. In theory, nudging the immune system in the other direction could partly work against what Kevzara is trying to do, which might mean less relief for your joint pain, swelling, and stiffness.

I want to be honest about the evidence here: this concern comes from lab and small human studies of the supplement, not from real cases of it weakening Kevzara. So it's more theoretical than proven. Still, before adding it, please run it by your rheumatologist or pharmacist so they can keep an eye on how well your RA stays controlled.

From our pharmacist’s overview on the Sarilumab page

“Sarilumab (Kevzara) works by quieting an overactive immune system in rheumatoid arthritis, and most of the supplements on this page push the other way. Immune stimulants such as ashwagandha, astragalus, and American ginseng could in theory blunt the drug's benefit and let joint symptoms creep back.”

About Rice Bran Arabinoxylan Compound

What is Rice Bran Arabinoxylan Compound?

  • Immune support
  • Support during cancer treatment
  • Boosting natural killer (NK) cell activity
  • General wellness in older adults
  • Fatigue and quality of life support

Rice Bran Arabinoxylan Compound (often sold under brand names like MGN-3 or BioBran) is a processed fiber from rice bran treated with mushroom enzymes, marketed mainly as an immune booster. Early small studies suggest it may affect certain immune cells, but high-quality human evidence is very limited, and it should never replace standard medical care. Talk with your pharmacist or doctor before using it, especially during cancer treatment.

About Sarilumab Injection

What Is Sarilumab Injection Used For?

Brand name: Kevzara

Kevzara (sarilumab) injection is approved for three conditions. In adults, it treats moderately to severely active rheumatoid arthritis (RA) when at least one other disease-modifying drug hasn't worked well enough or caused intolerance. It also treats polymyalgia rheumatica (PMR) in adults who haven't responded adequately to corticosteroids or who can't tolerate tapering them.

Kevzara is also approved for polyarticular juvenile idiopathic arthritis (pJIA) — a form of childhood arthritis — but only in patients who weigh 63 kg (about 139 lbs) or more. It is not approved for pediatric patients below that weight due to the lack of an appropriate dosage form.
