Theoretically, rice bran arabinoxylan compound might interfere with immunosuppressants by stimulating the immune system. In laboratory and human research, rice bran arabinoxylan compound has been shown to stimulate the activity of immune cells.
Some immunosuppressants include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the basic tension to understand. Azathioprine (Imuran) is prescribed to calm down your immune system, whether you're protecting a transplanted kidney or settling severe rheumatoid arthritis. Rice Bran Arabinoxylan Compound is marketed to do the opposite, to rev up immune cell activity. So in theory, the two could work against each other, and the supplement might blunt some of the benefit you're getting from azathioprine.

I want to be honest about the evidence here: this concern comes from lab studies and basic pharmacology, not from solid human trials showing real problems. Still, if azathioprine is keeping a transplant safe or controlling serious inflammation, even a small loss of effect isn't worth gambling on. Please loop in your transplant team, rheumatologist, or pharmacist before adding this product. Don't change your azathioprine dose on your own.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
